If you'd like to pre-print your own labels, you will need three labels per form.

A set of labels with dog's information: Registered Name, Call Name, Date of Birth,

Breed, Gender, Registration #, Sire & Dam Registration #s and Microchip/Tattoo ID #

AND a set with your personal information: Name, Co-Owner Name, Address,

Phone # and E-mail Address

PLEASE NOTE: A veterinary assistant or designated club member

will bring each dog in and return it to its owner when the exam is complete.
